Community Requests Form

Thank you for your interest in applying for CD14's Discretionary Funding. Please note that all community requests should be submitted at least 12 weeks before the event or project start date in which you are requesting CD14 financial support. Any submissions submitted after this deadline will be considered on a case by case basis. Requests not submitted through this portal cannot be considered.  

Please ensure to prepare the following documents for your application:

  • W9 
  • BTRC
  • Articles of Incorporation
  • Determination (501c3) Letter
  • Invoices/Itemized Budget (stating funding intent)

Note: If you are a business, please state that on the application and only the W9, BTRC and Invoice are required

Please be aware that due to budget constraints, our funding capacity is limited. Additionally, all allocations are processed as reimbursement after the funds have been spent by the receiving organization and will require proper documentation.
